3. Medical Information Disclaimer

The information provided on our website is for general informational and educational purposes only. It is not intended to be a substitute for professional medical advice, diagnosis, or treatment.

  • Always seek the advice of Dr. Sean Lazarus or another qualified healthcare provider with any questions you may have regarding a medical condition

4. Appointment Policy

To ensure we provide the best possible care to all our patients, please read and agree to the following:

Punctuality All patients are expected to arrive on time for each scheduled appointment. We will do our best not to keep you waiting.

24-Hour Cancellation Notice As a busy medical practice, we require at least 24 hours notice for any appointment cancellation or rescheduling. Failure to provide adequate notice disrupts your treatment progress and takes the appointment slot from another patient who may have needed it. A cancellation fee equal to the cost of the scheduled appointment may be charged for cancellations with less than 24 hours notice, with no exceptions.

No-Show Policy Patients who do not attend a scheduled appointment without prior notice may be charged the full cost of the appointment.

Block Booking We may recommend block booking appointments in advance when appropriate to your treatment plan. These appointments can be altered as needed and do not require payment in advance.

5. SMS and Communication Consent

By providing your phone number on our website forms, at our office, or through any other method, you consent to receive text messages (SMS), phone calls, and email communications from West Haven Foot and Ankle Center regarding:

  • Appointment confirmations and reminders
  • Follow-up care after visits
  • Responses to your inquiries
  • Missed call notifications

You may opt out of SMS communications at any time by replying STOP to any text message. Message and data rates may apply. For full details on how we handle your communications and data, please review our Privacy Policy.

6. Patient Responsibilities

As a patient, you agree to:

  • Provide accurate and complete information about your medical history, current medications, allergies, and symptoms
  • Follow the treatment plan recommended by Dr. Lazarus
  • Keep scheduled appointments or provide timely notice of cancellation
  • Pay for services rendered according to the payment terms agreed upon
  • Treat our staff and other patients with respect and courtesy

7. Payment and Insurance

Payment Payment is due at the time of service unless other arrangements have been made in advance. We accept cash, major credit cards, and certain insurance plans.

Insurance While we accept many insurance plans, it is your responsibility to verify your coverage and understand your benefits before your appointment. You are responsible for any co-payments, deductibles, and charges not covered by your insurance.

Outstanding Balances Unpaid balances may be subject to collection procedures. We reserve the right to refuse service to patients with significant outstanding balances until payment arrangements are made.
